You should know:
- After first three Doctors' Office Visits, all other visits are covered after the deductible.
- Maternity benefits are not included with this plan.

SmartSense Preventive Care
With SmartSense, annual physical exams and certain preventive screenings (routine mammogram, Pap and PSA tests) and well-child services are covered after you meet your deductible. You also have the option of going to a HealthyCheckSM Center which provides immediate coverage for annual preventive screenings.
